• Insurance Claims: Our office submits all claims directly to your insurance provider on your behalf.

• Understanding How Testing is Billed: Psychological evaluations are billed under two separate line items:

  1. Initial Evaluation

  2. Psychological Testing

• Patient Responsibility: Your specific copay, deductible, and out-of-pocket costs are determined entirely by your insurance carrier. Because coverage varies by plan, we strongly recommend calling your insurance company prior to your visit to ask what your  annual deductible is and whether it has been met for the year.

• Card-on-File Policy: We ask that you provide a credit card to keep on file in our secure system. This card will be used after we receive the Explanation of Benefits (EOB) back from your insurance carrier following claim submission.

• Payment Processing:

  • Balances under $200: We will automatically charge your card on file for the balance due as indicated by your insurance EOB.

  • Balances over $200: We will give you a courtesy call prior to charging your card to review the balance with you.
